Alberta Association of Animal Health Technology Privacy Statement
The following document has been created in accordance with the rights and obligations of Personal Information Protection Act for information collected after January 1, 2004.
Collection:
On application to the Alberta Association of Animal Health Technology (AAAHT) for membership we will request personal and business information. The AAAHT may also require personal information for other application/registration forms such as the VTNE, CE events, awards or investigations. Completion of forms in use will provide the basis for the information the AAAHT will hold in their custody. Information may also be collected by fax, phone, email or regular mail.
Information will be stored in the AAAHT database and/or in personal files for each member. If information is stored in other areas, such as closed disciplinary cases a notation will be placed in an individual’s personal file to aid in the access of information in the future.
The information is collected to aid in the governance of the Association. This information may be released to the General Membership in our Annual Directory. Select Graphix to distribute the newsletter and the AB.VMA to comply with the regulations of the Veterinary Professions Act. It may be disclosed to third parties for other purposes reasonably related to AAAHT business.
Personal information will be held current in the AAAHT office for the duration of an individual’s membership with the AAAHT. When membership has been cancelled at a member’s request or struck at the discretion of the Board of Directors personal information shall be archived in the AAAHT office for 5 years and then moved to off site storage (2 years). At the end of that designated period it may be destroyed.
Access:
Personal information is accessible by any of the Board of Directors or the staff contracted to facilitate the day to day running of the AAAHT for any reasonable purpose related and limited to AAAHT business who have been trained in the policies for protecting personal information and have taken an oath of confidentially.
To request access to your personal or business information held by the AAAHT forward a written request for such information. You will be responsible for any charges for copying or shipping the information. The AAAHT is required to respond to your request within 45 days of receipt of the written request. The AAAHT can and must refuse such an access request in part or in full if disclosure would invade the privacy of a third party or would interfere with a legal proceeding.
You may request your personal information be released with a written request to the AAAHT office. If an individual request information the AAAHT will make reasonable attempts to establish that individual is requesting on your behalf and/or to gain your consent to disclose information if it is required. There are instances where the AAAHT ay and is required to disclose personal information without the consent of individuals in question.
Updates To Personal Information:
You will be required to advise the AAAHT in writing of any changes to your personal or business information. Including but not limited to changes of address, place of employment, name or contact information such as but not limited to phone numbers or email addresses.
Each AAAHT member is also required to confirm their personal and business information annually when they renew membership status by returning the AAAHT Membership Roster Update.
If you believe that information the AAAHT has on file is incorrect you may send in a written notice requesting the particular information be corrected. The AAAHT Privacy Officer will review the request and determine if the information should be corrected. The AAAHT is obligated to respond with 45 days of receipt of the request for information correction.
Complaints:
The AAAHT Vice-President is designated as the AAAHT Privacy Officer. Any and all questions or concerns about the AAAHT policies regarding the handling of personal information should be directed to the Vice-President.
An individual that is not satisfied with the AAAHT’s response to their access request may ask the Alberta Information and Privacy Commissioner to review the matter.
